5010 278X217 Loop 2000E, DTP: DTP - ONSET OF CURRENT SYMPTOMS OR ILLNESS DATE
The TR3 states:
Required when the date of onset of the patient’s condition is different from the diagnosis date, and not accident or pregnancy related. If not required by this implementation guide, do not send.

Could I get further clarification on this situational rule. What diagnosis date is used here? For example, if a patient has a chronic disease, should the original diagnosis date be used? Or, is it the diagnosis date for the current episode of care?

This issue is explicitly addressed in guide 005010X217. The 2000E loop HI segment contains twelve composite data elements. Each composite conveys a diagnosis code as well as a date related to that diagnosis code (component elements 03/04) with a situational rule that reads "Required when the date diagnosed is known. If not required by this implementation guide, do not send."
Therefore, the operative date(s) is(are) the date that the specific diagnosis conveyed in each of the HI segment composites were identified. The onset date is only conveyed when different than any of those dates.
