2010E NM1 "Use this segment to convey the name and identification number of the service provider (person, group, or facility) or to identify the specialty entity." The required NM101 allows codes for the first half of the condition (service provider, facility, group, clinic, person) but I don't see a code to identify a specialty entity. I am confused because Service Provider is the name of the segment as well as a code choice description. How are specialist and specialty entity indicated?

This is a generic Service Provider Segment. A Service Provider is one who performs a service. This can be a general practitioner or a specialist. The code 1T accommodates all types of physicians, clinics or group practices and the code FA accomodates all types of facilities.
The specialty entity can be identified using the Provider Taxonomy (PRV), or based on the UM03 value in the UM segment of the loop 2000F associated with this loop 2000E.
