If the Medical Record number submitted in the 276, is it a HIPAA mandate that this data be sent back out on the 277?


Here is the segment in the 276:

Loop: 2200D — CLAIM SUBMITTER TRACE NUMBER

1515 Notes: 1. This is the Medical Record number submitted on the original claim

and should be sent when available from the submitted claim.

1802 2. Use this only if the subscriber is the patient.

105009 3. The total number of REF segments in the 2200 Loop cannot exceed 3.

Response

The 004010X093A1 specifically addresses use of the Medical Record REF Segment in the 277. The Segment notes indicate the Medical Record should be sent when available from the submitted claim, regardless if the value was or was not sent in the 276. If the 277 REF segments exceed the maximum of 3, it will be at the discretion of the Information Source which 3 REF segments are returned.
